At the end of each plan description contained on this site, there is a section that explains how to apply for benefits. If you or your beneficiary apply for benefits and either part or all of the request is denied, you have the right to appeal that decision. The following procedure applies:

  • If a claim for benefits is either wholly or partially denied, you will be notified in writing. The notice will state the reasons why the claim was denied.
  • After receiving the notice, you or your beneficiaries have 60 days to request, in writing, a review of your claim. You are entitled to review all documents pertaining to your claim and may also submit comments pertaining to your claim.
  • Those receiving your request have 60 days to set up a hearing that you and your representative, if you wish, may attend.
  • You will be informed about the final decision regarding your claim as soon as possible, but in no case later than 120 days after your request for review is received.
