Enrolling in the Dental Health Plan
Changing or stopping coverage

Participation in the Boston University Dental Health Plan is voluntary. You may elect this coverage within 30 days of your new employee benefits orientation meeting, during the annual open enrollment period, or if you have a qualified change in your family or employment status. To elect this coverage, you must complete an enrollment form. Enrollment forms are also available from the Benefits Section of the Office of Human Resources. This form will also authorize a payroll deduction to pay for your share of the cost.

If you choose a family membership, coverage is guaranteed only for the family members who are listed on your enrollment form. If you wish to enroll newly eligible family members (for example, an adopted child or a new spouse), you may do so by obtaining the necessary forms from the Benefits Section of the Office of Human Resources.
