Appealing a denied claim

Following are procedures for obtaining benefits if your provider is not affiliated with a Boston University Dental Health Center:

  • Obtain a Dental Health Plan claim form. Claim forms are also available from the Benefits Section of the Office of Human Resources.
  • Pay your dentist for services.
  • Submit your claim form with original bills and receipts to:

  • Boston University will review your claim form, then reimburse you for covered services according to the appropriate percentage of the standard fee schedule.

Note: You must use the pre-treatment estimate whenever a planned course of treatment is expected to exceed $300.

A separate claim form must be completed for each covered plan member. Claims must be submitted within six months of the date of treatment.

If your dependents are covered under their health plan for basic dental health benefits, then the health plan provides the primary benefit and this plan, the secondary benefit.
