Biographical Data Form
Following is a list of the data elements that are required to be completed:
- Name
Use the format last, first, middle initial.
- Social Security Number
This will be used to generate a University employee identification number.
- Salutation
Indicate the preferred salutation of the candidate, e.g. Dr., Mr., Professor, etc.
- Home Address
Please provide the current local address. If the faculty member is relocating to Boston, please indicate the date through which their home address is valid. Once the faculty member is set up on the personnel/payroll turn-around form, we will reference this file for address information.
- Home Phone
As with "address," please indicate the date through which this number will be valid.
- United States Citizen
Check the appropriate box. If "no," list visa type and expiration date. Please consult with the International Students and Scholars Office whenever proposing faculty who are not U.S. citizens or permanent residents. It is imperative that Boston University complies with all Department of Justice rules and regulations concerning the issuance of international student, scholar and visitor visas. Failure to do so could jeopardize the University's ability to issue such visas.
- Education
List all degrees, year received (or expected date of completion), and institution. List the degrees in the order of highest degree first. Do not use abbreviations except for degrees. Please include the location of the institution (city and state).
- Prior Boston University Work Experience
Please list any previous work at Boston University.
- Work Experience from Completion of Terminal Degree
Please list those items of experience that are most relevant to the appointment being recommended. This information will be provided in the Board of Trustees' summary. Do not indicate "see attached vita." Please include the location, including city and state.
- Form Completed By
Signature of person completing form and who will be contacted if there are any questions.
