Dean's Evaluation

Please see that all questions addressed to the candidate and Chair are answered and that all supporting material has been submitted as requested.

Following is a list of data elements that must be completed:

  1. Name
    Use the format last, first, middle initial.
  2. School/Department
    List the academic unit and department name. Commonly used acronyms are acceptable.
  3. Academic Year of LOA/SAB
    Indicate the academic year for which the LOA/SAB is proposed.
  4. Total Released Salary
    Indicate the salary amount (if applicable) that will be released.
  5. Dean's Evaluation of Proposal
    In the space allotted on the form, an evaluation of the candidate's proposal, and an assessment of the value of the proposed leave or sabbatical to the College and University should be provided by the Dean.
  6. Dean's Evaluation of Arrangements for Course Coverage
    If the Dean approves of the department Chair's strategy for coverage of the candidate's teaching, this should be confirmed. If the Dean does not approve it, a written commentary should be sent to the Chair and the faculty member. The Dean should also address the coverage of the candidate's College or University-wide responsibilities and report on the search for external funding.
  7. Signature
    The Dean's signature is required on the appropriate line of this form as well as the Faculty Request form. Also, concurrence or dissent of the leave or sabbatical request, as well as concurrence or dissent of the request for deferment of the tenure review year if applicable, should be checked. Denials of leaves or sabbaticals by the Chair and/or Dean should still be forwarded to the Provost.
