Full-Time Faculty Reappointments/Non-Reappointments

The Recommendation for Full-Time Faculty Reappointment/Non-reappointment form is required when recommending the reappointment/non-reappointment for all non-tenured full-time faculty (including tenure track).

The reappointment paperwork for March and December is due in the Provost's Office one month before the notification date. The reappointment paperwork for the August review is due mid-April. See the Deadlines for Faculty Actions/Academic Services. The Provost's Office will provide an updated schedule each academic year.

The reappointment notification date is assigned according to the following schedule.

  • For faculty in their first year of service, notification of reappointment or non-reappointment is generally received on or before March 1.
  • For faculty in their second year of service, notification of reappointment or non-reappointment is generally received on or before December 15.
  • For faculty in their third or greater year of service, notification of reappointment or non-reappointment should be received on or before August 31 of the year preceding the last year of their appointment.

Please note: The reappointment notification date is calculated on the full-time date of hire for every subsequent reappointment. For example, a faculty member appointed initially for a two-year contract, and subsequently for another two year contract, would be on the August cycle with notification a full year before contract end-date. This reappointment date would be based on the fact the faculty member was appointed full-time for three or more years.

Research faculty appointments are contingent on grant funding and therefore do not require a reappointment notification date. Part-time and Visiting faculty also do not require a notification date.

"Terminal" appointments, in which the faculty member will not be considered for a reappointment, need to be stated in writing to the faculty member by the Dean when they are appointed. Similarly, faculty who are hired from year to year on an as-needed basis and thus will not be reviewed under the University reappointment cycle for full-time faculty must also be advised of this in writing when appointed/reappointed.

Please note: For tenure-track faculty: their current contract should not end in their tenure review year. All faculty must be reappointed one year short of the tenure review year or one year beyond the tenure review year to include the terminal year should they not be awarded tenure.

If the reappointment is approved, your Faculty Actions Analyst will fill in the bottom part of page 1, indicating the length of the new contract, whether the next review is a reappointment review or tenure review, and all applicable dates.

The following items are required for the reappointment package to be complete:

  1. The completed and signed, two-page recommendation forms for Full-time Faculty Reappointment/Non-Reappointment.
  2. A current vita (no more than three months old).

Use Adobe Acrobat to open, complete, and save the form so you can go back to it. Print the completed form and obtain appropriate signatures on the printed copy. Forward the signed copy of the form, with relevant supporting materials, to the Office of the Provost.

All sections of the form MUST be completed. Areas that do not apply should be marked "N/A." Please attach additional pages if more space is required for listing the history of courses taught.

Following is a list of the data elements that are required to be completed.

Page One

  1. Name
    Use the format last, first, middle initial.
  2. University Identification Number
    List the faculty UID number.
  3. Date Form Completed
    Use the format month/day/year.
  4. School-Department
    List the academic unit and department name. Commonly used acronyms are acceptable. If the appointment is joint, please specify both schools and departments.
  5. Rank/Title
    Indicate the current rank (and administrative title, if applicable).
  6. Initial Full-Time Appointment Date
    Since this date determines the next review date, it is important to indicate the initial date of full-time faculty appointment.
  7. Current Appointment Period
    Use the format month/day/year to indicate "from" and "to." This date refers to the faculty's most current contract, not the proposed contract term.
  8. Projected Tenure Review Year
    Indicate the year in which the faculty member will be reviewed for tenure, if applicable. If not applicable, put "N/A" in this box.
  9. History of Courses Taught
    Provide the detailed information on course name, title, number, credit hours and actual enrollment as requested, listing the initial year/semester first. Please attach additional pages only if more space is required for listing the history of courses taught. If additional space is necessary, please use the Continuation Form (PDF).
  10. Recommendation/Approval
    The department Chair and Dean should indicate reappointment or non-reappointment by checking the appropriate box. If reappointment is recommended, fill in the number of years of reappointment requested.
  11. Signatures
    The first page of this form is to be signed and dated by the department Chair and Dean. For joint appointments, the signatures must be obtained from both units and/or departments. The Provost will then check the appropriate box, sign and date.

Page Two

Allotted space for the evaluation of research, teaching and other professional activity by the Chair and Dean is provided on the second page of the form. This evaluation should include comments about the need for the position and future need/role of the faculty member.
