Secondary Reappointments

When faculty with secondary administrative appointments are being reappointed for an additional year(s), use the Recommendation for Secondary Academic Reappointment Form (PDF).

The complete reappointment paperwork package should include:

Use Adobe Acrobat to open, complete, and save the form so you can go back to it. Print the completed form and obtain appropriate signatures on the printed copy. Forward the signed copy of the form, with relevant supporting materials, to the Office of the Provost.

Following is a list of the data elements that are required to be completed:

  1. Name
    Use the format last, first, middle initial. Please include exactly as it is written on the candidate's vita.
  2. University Identification Number
    List the faculty UID number.
  3. Academic Year
    Indicate the Academic Year for which the reappointment is proposed.
  4. School/Department
    List the academic unit and department name. Commonly used acronyms are acceptable.
  5. Title
    List the proposed title of the recommended secondary reappointment, e.g. Chair, Director, etc.
  6. Current Rank
    List the current faculty rank held by the candidate being recommended for a secondary reappointment.
  7. Reappointment Period
    Use the format month/day/year for start- and end-date of the reappointment.
  8. Compensation
    The compensation recommended for administrative appointments varies, and typically includes an administrative stipend and/or course release.

    For an administrative stipend, there may be a

    • set amount (ex. $3,000)
    • percentage of base (ex. 1/9th of base)
    • a combination of the above

    For the course release, there may be a

    • reduction from teaching while serving
    • reduction from teaching at a later date
  9. Funding Source
    List all budgeted funding sources and amounts, e.g. unit-department-object code-source.
  10. Work Assignment
    List courses to be taught by year and semester; do not use abbreviations for course names. Indicate the course title, course number, credit hours, and estimated enrollment. If additional space is required, please use the Continuation Form for History of Courses Taught (PDF).
  11. Secondary Appointment Duties
    List all duties that will be entailed in the secondary appointment.
  12. Chair's Evalution (if applicable)
    In the space allotted on the form, provide a brief statement from the Chair of the department (if applicable) which states why a secondary reappointment is being recommended for the faculty member.
  13. Dean's Evaluation
    In the space allotted on the form, provide a brief statement from the Dean which states why a secondary reappointment if being recommended for the faculty member.
  14. Replacing (if applicable)
    Indicate by name and position number the faculty member whose position is being replaced. If this is a new position, so indicate.
  15. Signatures
    The Chair (if applicable) and Dean must sign and date the form. A signed copy will be returned as official administrative approval.
